When reviewee and reviewer meet prior to the observation to establish the focus of the review and the logistics the reviewee will need to complete the Information for Faculty form.

Both these templates are the basis of the peer observation report and help form the feedback to be provided. It can also help guide the reviewee in developing a focus and session objectives for the review.
The reviewer should decide which of the templates most suits their needs.
As a result of the peer observation and feedback both the reviewee and reviewer will be reflecting and goal setting the Peer Review Reflection Sheet will assist with this process
